5 years of experience in the last 7 years working with individuals with IDD and behavioral/mental health concerns.
Proven track record in developing and implementing Individual Program Plans (IPPs).
Experience linking individuals with IDD to community supports and resources.
Experience with clients having criminal and forensic involvement.

Key/Essential Functions & Responsibilities
Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
Facilitate and lead Intake/admission meeting.
Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans
Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director
Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan
Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.
Performs all other duties, as assigned.
Annual Salary: $87,560.30
Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)
