Director of Clinical Delivery for IHCIThe Director of Clinical Delivery for IHCI provides strategic and operational leadership for clinical care management services, overseeing nursing, social work, medical assistant, and care management teams. This role is responsible for driving population health initiatives, value-based care performance, clinical quality outcomes, patient and provider satisfaction, and operational excellence across multiple markets.
The Director will lead clinical transformation initiatives, oversee transitions of care and post-acute programs, and collaborate with healthcare partners to support successful value-based care strategies and sustainable clinical operations.
What You'll Do- Lead day-to-day and strategic clinical operations for care management programs.
- Oversee multidisciplinary teams, including RNs, Social Workers, Medical Assistants, and care management staff.
- Drive population health, value-based care, transitions of care, and post-acute care initiatives.
- Partners with physicians, health systems, and executive leaders to improve quality, patient experience, and financial performance.
- Monitor program outcomes and utilize data to identify opportunities for improvement.
- Lead change management efforts and support clinical transformation initiatives.
- Manage budgets, staffing plans, and operational performance.
Exceptional Skills and Qualifications- Bachelor's Degree in Nursing, Social Work or other related discipline is required.
- Master's Degree in Business, Nursing, Social Work, or other related discipline is preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) or equivalent certification preferred.
- If a nurse, licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) is required.
- Five (5) or more years of clinical staff management experience in population management or managed care/insurance or hospital case management is required.
- Five (5) or more years of operational management in a clinical setting is required.
- Five (5) or more years of experience leading teams that Support Medicare ACO populations, and organizational effectiveness is preferred.
- Strategic thinking and the ability to execute complex initiatives.

- Remote work with occasional in person meetings per year. Prefer candidate living within central or southern Indiana. Candidate must be willing to travel between central and southern Indiana markets.
