Clinical Manager - Home HealthPay Range: $110k - $130k annually

Job SummaryThe Clinical Manager oversees home health clinical operations, ensuring high-quality patient care, regulatory compliance, and effective care coordination. This role leads an interdisciplinary team and ensures services align with physician orders and individualized care plans.
Key Responsibilities- Manage clinical staff and daily operations in a home health setting
- Develop and oversee plans of care (POC) and patient outcomes
- Review referrals and assign clinicians based on patient needs
- Ensure compliance with CMS, Medicare, and agency standards
- Conduct chart audits, case reviews, and case conferences
- Collaborate with physicians and support care coordination
- Participate in QAPI and performance improvement programs
- Assist with hiring, training, and staff development
Qualifications- Licensed RN, PT, OT, SLP, or MSW (active state license)
- 2+ years home health experience required
- Supervisory or leadership experience preferred
- Strong knowledge of home health regulations & documentation
