Job Summary

The Department of Mental Health, Substance Abuse and Developmental Services seeks a Support Coordinator with the Developmental Services Division. This position is responsible for assessing, coordinating, monitoring, and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid is preferred. Additionally, knowledge of community-based resources including integrated housing options and employment first principles is desired.

Duties may include:
  • Assessing eligibility and the needs of individuals.
  • Assessing possible individual risks.
  • Interviewing individuals and their families.
  • Facilitating team meetings.
  • Coordinating service delivery among providers and developing and documenting person-centered plans using a strengths-based approach.
  • Assuring quality service provisions.
  • Maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention.
The successful candidate must:
  • Demonstrate effective oral and written communication skills.
  • Have strong organizational and time management skills.
  • Have proficiency in utilizing electronic health records and other software programs for documenting service delivery.
The Support Coordinator is responsible for providing high-quality customer service consistent with the mission, vision, and values of MHSADS. Hiring salary commensurate with experience.
Minimum Qualifications

Bachelor's degree in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field and three (3) years of related experience in assigned program area or equivalent combination of education and experience.

Preferred Qualifications:
  • Master's degree in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field.
  • Three (3) years of related experience in assigned program area as a casemanager, service coordinator, or multi-disciplinary team facilitator with children, youth, adults, and families.

Job Contingencies and Special Requirements

Employment is contingent upon successful completion of background checks, including fingerprinting, NCIC, credit, and DMV checks. The successful candidate must have a valid driver's license in Virginia or regional state of residence.
