Pay Range: $44.70-$51.40Schedule: Monday-Friday What You Bring (Requirements)- A minimum of one year's experience in working with the frail or elderly population required.
- Graduate of an accredited school of professional nursing
- Current state Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required
- Must have a valid driver's license, proof of insurance and have means of transportation.
What You'll Be Responsible For:Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant's homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in-person nursing re-assessments in the PACE clinic and in participant's homes semi annually and as needed.
- Participates in the nursing on-call rotation.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
