How Your Territory Rotations Work

You'll rotate throughout our service area based on patient census, staffing, and clinical coverage needs.

A typical territory assignment may last around three months.

During that rotation, you'll primarily work within that geographic area and manage an assigned patient caseload.

That gives you time to:

  • Learn the territory and develop an efficient routine
  • Become familiar with your patients and caregivers
  • Build relationships with the clinical team
  • Understand the needs of that patient population
  • Provide continuity throughout episodes of care
  • Become a dependable resource within the territory

When organizational needs change, you'll transition to another area and begin building that same familiarity there.

It's the variety of floating with enough consistency to actually become effective.

What You'll Do

Regardless of which territory you're supporting, you're still the RN Case Manager responsible for helping lead your patients' care.

You'll:

  • Manage an assigned home health caseload within your current territory
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing visits
  • Perform wound care, medication management, and other skilled nursing services
  • Educate patients and caregivers
  • Monitor progress throughout the episode of care
  • Identify changes in condition and intervene appropriately
  • Coordinate care with physicians and other healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help reduce avoidable hospitalizations and support patients in remaining safely at home
  • Maintain timely and accurate documentation in Homecare Homebase (HCHB)
