Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

What You'll Be Doing

  • Provide skilled home health nursing visits in patients' homes
  • Complete comprehensive nursing assessments and ongoing evaluations
  • Perform wound care, medication management, injections, and other skilled nursing services
  • Monitor acute and chronic medical conditions
  • Identify changes in condition and intervene appropriately
  • Educate patients and caregivers about medications, diagnoses, treatments, and disease management
  • Coordinate care with physicians and other healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help patients safely transition home following hospitalization or rehabilitation
  • Complete timely documentation using Homecare Homebase (HCHB)
  • Help reduce unnecessary emergency department visits and rehospitalizations

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
  • Strong nursing assessment and critical-thinking abilities
  • Confidence making clinical decisions independently
  • Excellent patient and caregiver communication skills
  • Strong organization and time-management abilities
  • Ability to manage a daily schedule of patient visits
  • Reliable transportation and willingness to travel throughout the Western Twin Cities territory

Previous home health experience is valuable, but it isn't the only path to becoming a strong Visit Nurse.
