As the Registered Nurse, you will be responsible for providing direct, and indirect, services to individuals served within the ACCS program. The Registered Nurse provides medical services and coordinates the care planning process and overall provision of services to the individuals served in the program.
- Provides oversight of MAP, medical and psychiatric monitoring, consultation and coordination for individuals served.
- Actively involved in assessment, treatment planning and service delivery processes and responsible for determining individuals' need for rehabilitative services as required by Medicaid Rehabilitation Option
- Acts as liaison to area and regional medical facilities in cooperation with Program Directors.
- Provides ongoing in-service training and consultation to other staff members and families on issues of medical care/health as they relate to developmental disabilities.
- Serves as a resource to individuals and families.
- Participates in clinical assessments by preparing and submitting nursing assessments and progress notes in accordance with agency policies.
- Writes health history/status and self-care sections of evaluation reports, when requested.
- Makes recommendations to be written and included in individuals' plans for remediation of health/nutritional deficits when appropriate.
- Accompanies individuals to appointments as agreed with Program Directors and/or Senior Program Manager.
- Convenes regular health care management meetings to ensure individuals are receiving necessary care from health care providers.
- Other tasks as assigned by program leadership
Who You Are- Active Registered Nurse license in the state of Massachusetts
- Must have valid driver's license and a reliable car available for working hours to transport individuals served
- Minimum 6 months in Human Service field preferred
Pay: $40 - $42.50 per hour
