In this dynamic role, you will combine clinical expertise, care coordination, advocacy, education, data management, and program development to help patients receive seamless, timely, and personalized care.
ResponsibilitiesWhat You'll Do
- Coordinate comprehensive care for patients with ALS and muscular dystrophy.
- Serve as a consistent point of contact for patients, families, providers, and community resources.
- Navigate complex, high-acuity clinical needs across multiple specialties.
- Facilitate appointments, referrals, testing, treatment planning, and follow-up care.
- Provide patient and family education, support, and advocacy.
- Collaborate with neurologists, therapists, social workers, respiratory specialists, and other members of the multidisciplinary team.
- Collect, analyze, and maintain program and patient-care data.
- Create reports, presentations, and quality-improvement materials.
- Provide administrative support for program operations and clinical workflows.
- Participate in research, grant development, and program-improvement initiatives.
- Help build innovative services that improve access, continuity, and outcomes.
What You Bring.
- Experience with care coordination, patient navigation, or complex case management.
- Neurology, neuromuscular, ALS, or muscular dystrophy experience strongly preferred.
- Ability to manage high patient acuity with compassion, confidence, and sound clinical judgment.
- Experience with data collection, reporting, and presentations.
- Strong communication, organization, problem-solving, and relationship-building skills.
- Research experience and grant-writing knowledge are a plus.

QualificationsEducation
- Bachelor's Degree Bachelor's of Science, Nursing - minimum
- Master's Degree Master's of Science, Nursing - preferred
Experience
- 3 years Experience in an acute care setting (transplant, cancer, breast, pre-natal); preferred experience with program development and implementation of navigator programs. - minimum
Licenses and Certifications
- RN State License - Upon Hire minimum Registered Nurse
- ANCC Specialty Certification - 1 Year minimum Professional Certification in area of specialty
