Qualifications

• One year of home health case management experience preferred.
• Minimum of one year of nursing experience required; two or more years preferred.
• Supervisory experience or mentoring of nurses highly preferred.
• Current and unrestricted California RN license required.
• Current BLS/CPR certification required.
• Valid California driver’s license required.
• High level of clinical knowledge in home health care is key.

Responsibilities

• Create, implement, and execute overall patient care plans to promote wellness.
• Conduct comprehensive patient assessments including Start of Care, evaluations, and discharge planning.
• Oversee 2 nurses in executing care plans and provide guidance.
• Educate patients and families on wound care, medication management, and other medical needs.
• Develop advanced plans of care using assessment data, including rehabilitation strategies.
• Collaborate with interdisciplinary teams and physicians to enhance patient care.
• Prepare clinical documentation and ensure compliance with timelines.
