Job Description:
Manages Community Care program operations and ensures compliance with clinical standards, regulatory requirements, and quality monitoring processes.
Monitors productivity and outcome metrics, identifies root causes of performance issues, and implements corrective actions.
Ensures accurate, timely, and comprehensive documentation of patient care plans and responses in medical records while maintaining confidentiality and legal compliance.
Prescribes, delegates, and coordinates patient care activities to achieve established care plans and priorities.
Develops and fosters relationships with physicians, clinics, universities, and community partners to support program goals.
Implements strategic plans and provides input on program development and budgetary needs.
Oversees hiring, scheduling, orientation, training and performance management of Community Care staff.
Attends and conducts team meetings, case conferences, and provides guidance for care improvement.
Considers safety, effectiveness, and cost factors in service delivery and resource management.
Performs other duties as assigned.
