Job Summary: Under general direction, provides psychosocial assessments, clinical interventions, discharge planning, referrals to other facilities, and interfaces with families and community case managers. Performs related duties as required.
DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
1. Attend daily rounds and be able to give reports to multi-disciplinary team. Cover other units as assigned.
2. Carry a caseload of eleven to twelve patients, charting minimum of two progress notes per patient. Run groups of three to twelve patients using solution focus model. Keep and provide a timely and accurate daily patient activity log.
3. Interviews patients and relatives to gather social and psychiatric data and participates in developing treatments and planning for the casework programs of patients.
4. Meets with patients for regular therapeutic interviews. Guides the patient in understanding their illness and contributing personal, social, and economic factors. Assist patients in developing realistic plans and advises patients on community resources. Makes referrals and other arrangements as necessary.
5. Prepares reports, case summaries, and correspondence as required; maintains case histories and work records, including required statistics.
6. Works with relatives and community agencies of patients to elicit their cooperation in treatment planning and discharge planning.
Qualifications
Education: Master's Degree from an accredited school of Social Work with field placement in a psychiatric setting.
Minimum Experience: Equivalent of one year of experience in the class of Psychiatric Social Worker I; or the equivalent of two years of paid post-master's experience as a Psychiatric Social Worker in an outpatient facility, hospital, or other clinical setting.
Required Licenses/Certifications: CPI -Crisis Prevention Intervention Training (required for all positions at John George Psychiatric Pavilion; and certain positions in the Emergency Department).
Required Licenses/Certifications: Registered with the California State Board of Behavioral Science Examiners as an Associate Clinical Social Worker (ASW) or possession of a current license as a Clinical Social Worker (LCSW).
Pay Range $86.48
The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization.
