Location: Salem Hospital, 890 Oak Street SE, Salem, Oregon 97301
Department: Pharmacy
Position Type: Full Time, 40 hours/week, Non-Exempt
Schedule: 24-hour float coverage, Four rotating 10-hour shifts

Your role with us

As a pharmacist at Salem Health, you will provide expert medication management and therapy optimization for hospitalized and ambulatory patients, collaborating with healthcare teams to ensure safe and effective medication use. You will also supervise and mentor pharmacy staff, provide educational programs, and participate in quality management initiatives. Strong clinical skills, excellent communication, and a commitment to patient-centered care are essential for success in this role. This position provides 24-hour float coverage across four rotating 10-hour shifts, ensuring continuous support for patients and teams while maintaining work-life balance.

What you'll do

  • Evaluate patient medical records, including medication use, allergies, and laboratory data, to ensure safe and effective medication therapy.
  • Verify provider medication orders for accuracy and appropriateness, making recommendations for therapy changes as needed.
  • Monitor and manage ongoing medication therapy for assigned patients, collaborating with prescribing physicians to resolve problems and optimize therapeutic results.
  • Provide medication information, drug therapy options, and pharmacokinetic dosing advice to medical personnel.
  • Supervise and direct pharmacy staff, including certified pharmacy technicians, pharmacy interns, and pharmacy students.
  • Serve as a liaison to nursing units or departments, managing inventory and providing educational programs as needed.
  • Participate in quality management initiatives, patient-care rounds, and hospital committees to promote best practices and optimal patient outcomes.
  • Maintain expertise and continuing competence in practice areas through ongoing education and self-study.

Required qualifications
