Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Valid Pennsylvania Pharmacist License with Injectable License for vaccine administration.
• Experience with Outcome Platforms is necessary.
• Medication Therapy Management (MTM) certification is preferred.
• Board Certified Ambulatory Care Pharmacist (BCACP) certification is beneficial.
• Current CPR certification required.
• Valid PA Driver's License with a clean driving record.
• Spanish language proficiency is advantageous.

Responsibilities

• Provide comprehensive medication management services to patients.
• Collaborate with healthcare teams to optimize patient care.
• Administer vaccines as per protocols and guidelines.
• Conduct medication therapy management consultations.
• Educate patients on medication use and health management strategies.
• Document patient interactions and maintain comprehensive records.
• Participate in quality improvement initiatives and training sessions.
