Full Job Description
As a Pharmacist, you will play an essential role in ensuring the safe and effective use of medications for members & patients within a Long Term Care (LTC) pharmacy setting. Your primary objective will be to dispense prescription medications accurately while providing expert advice on their proper use, potential side effects, and interactions. You will serve as a trusted healthcare professional, collaborating with patients, healthcare providers, and pharmacy staff to optimize therapeutic outcomes. Additionally, you will be responsible for helping to maintain compliance with all regulatory requirements and pharmacy policies to uphold the highest standards of member & patient safety. Your role will also involve helping to managing inventory, using your knowledge to interact with medical providers and members or patients and contributing to health promotion initiatives within the community.

Minimum Qualifications:
  • Bachelors or Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Valid and active pharmacist license in the state of practice within the United States.
  • Strong knowledge of pharmacology, drug interactions, and pharmacy law.
  • Experience with pharmacy management systems and electronic health records.

Preferred Qualifications:
  • Previous experience working in an LTC pharmacy setting.
  • Experience using framework LTC and ECM pharmacy systems.
  • License immunizer.
  • Familiarity with medication therapy management (MTM) services.
  • Additional training or certification in specialized areas such as diabetes education or anticoagulation management.
  • Bilingual in English and Spanish to serve diverse patient populations.
