Full Job Description
Physician Assistant/Nurse Practitioner, 5 Days a Week, 8 AM to 4 PM

Position Summary: "Empowering Elders, Enriching Lives"

Under the Medical Director's supervision, the Doctor/MD, PA or NP provides primary care, performs assessments, develops care plans, treats participant complaints, educates participants and caregivers, and collaborates with the Interdisciplinary Team (IDT) to care for frail elders according to Every Age PACE standard.

Essential Duties and Responsibilities:

  • Team Collaboration: Participates in meetings, communicates changes, and collaborates on care plans.
  • Participant Care: Conducts assessments, exams, and integrates care plans.
  • Acute and Long-term Care: Manages illnesses, nursing home care, and discharge planning.
  • Administrative Tasks: Maintains records, handles coding, orders lab tests, and ensures safety.


Minimum Education:

  • Bachelor's Degree (Required)
  • Doctor/MD: Doctorate Degree (Includes PhDs): MD or DO
    • Board Certification - Internal Medicine or Family Practice
    • Board Certification - Geriatrics preferred
    • Current North Carolina physician license
    • Current BCLS
  • North Carolina certification and DEA Registration
  • Physician Assistant: Graduate of certified or accredited Physician Assistant Program
  • Nurse Practitioner: RN license and Nurse practitioner license with prescriptive authority preferred.
  • CPR certification (Required)


Minimum Requirements:

  • A minimum of one year's experience in working with the frail or elderly population (Required)
  • Two years' experience as MD/Physician Assistant/Nurse Practitioner in a geriatric setting (Preferred)
