Job Description Summary:
Monarch is searching for a Nurse Practitioner or Physician Assistant for an Outpatient Primary Care position in both Wilson and Rocky Mount, NC. This will be a hybrid role of two days onsite at each location and one day remote from home. Work Schedule: Mon-Fri 8am-5pm. There will be occasional travel for home / community visits as needed for medical necessities averaging one day per month.

The Primary Care Provider (Advanced Practice Provider - Physician Assistant or Nurse Practitioner) delivers high-quality medical care in the Monarch Healthcare clinic locations and within the community for home visits as needed.

Responsibilities include conducting assessments, managing chronic conditions, prescribing and coordinating care, and promoting preventive health practices. The APP collaborates closely with Monarch Healthcare staff, behavioral health clinicians, and community partners to ensure integrated, whole-person and high-quality care is delivered. The position emphasizes outreach, relationship-building, and continuity of care to reduce barriers, improve wellness, and minimize avoidable hospital and emergency department use.

What You'll Do:

• Deliver Primary Care in the Clinic and Community: Provide assessments, preventive screenings, chronic disease management, acute care, and urgent interventions in the clinic and home/community settings.

• Clinical Management: Conduct physical exams, review prior treatments, order diagnostic tests/referrals, prescribe medications, and develop person-centered care plans in accordance with best practices and protocols.

• Education & Engagement: Provide health education to individuals and families, supporting understanding of diagnoses, treatment options, and preventive practices.

• Collaboration & Coordination: Partner behavioral health providers, hospitals, outside primary care clinics, public health departments, and community agencies to ensure integrated, whole-person care.

• Continuity of Care: Support transitions from hospital or emergency settings back into the community and participate in care planning for high-risk or complex individuals.

• Documentation & Compliance: Maintain timely, accurate records in the Electronic Health Record (EHR); adhere to all agency policies, procedures, and training requirements.

• Professional Development: Attend required meetings and trainings; maintain certifications and licensure.

• Other Duties: Perform additional responsibilities as assigned; travel required for community-based care delivery.
