Full Job Description
Location: Tampa/Clearwater, Florida Schedule: Monday through Friday, 8:00 AM to 5:00 PM Work Setting: Field-based, in-home patient care with teleconference and telephone support as appropriate. This is not an office-based or clinic-based position. Description: The Nurse Practitioner will serve as part of an interdisciplinary care team supporting a small panel of patients with chronic kidney disease, including CKD stages 3b, 4, 5, and ESRD. This position focuses on delivering high-quality, value-based care through in-home visits, teleconference, and telephone interactions as appropriate. The Nurse Practitioner will work closely with patients, caregivers, and care team members to coordinate comprehensive care, address medical and psychosocial needs, and improve patient outcomes. What You Will Do:
Assume medical responsibility for an assigned small panel of patients in collaboration with the care team.
Provide clinical services focused on patient-level and system-level interventions, including medical and kidney-focused care.
Complete patient histories and physical examinations.
Order laboratory and diagnostic tests as clinically appropriate.
Review and evaluate laboratory and diagnostic test results.
Educate patients regarding medication use and potential side effects.
Prescribe medications as permitted by applicable state prescribing authority.
Identify and manage medical conditions and barriers to care.
Participate in frequent interdisciplinary care team huddles.
Educate patients and caregivers regarding medical conditions and treatment plans.
Determine when consultation with medical, surgical, or psychiatric specialties is appropriate.
Coordinate and facilitate care across settings including emergency departments, hospitals, skilled nursing facilities, home health agencies, hospice organizations, and community-based organizations.
Perform structured patient assessments.
Develop and implement comprehensive care plans for assigned patients.
Provide after-hours telephonic support as needed.
Accurately document medical encounters in a timely manner.
Assist with the development and implementation of clinical protocols and care guidelines designed to optimize patient outcomes.
Maintain confidentiality of all patient-sensitive information.
Support continuous quality improvement initiatives.
Follow established organizational policies, procedures, and operating standards.
