  • Perform ongoing case evaluations through home visits, interdisciplinary meetings, and record review.
  • Guide and mentor clinicians to promote high-quality, compliant hospice services.
  • Assist clinicians in developing care plans, establishing goals, and prioritizing interventions.
  • Lead and participate in interdisciplinary team case conferences.
  • Conduct quarterly clinical record reviews and communicate findings to leadership.
  • Support recruitment, interviewing, and onboarding of new clinical staff.
  • Assist with planning and delivering continuing education, staff training, and in-service programs.
  • Support policy and procedure development and implementation.
  • Ensure compliance with professional standards, hospice regulations, and organizational policies.
  • Assess community health needs and support program development to meet those needs.

Position Summary

The Clinical Manager oversees the coordination, quality, and delivery of hospice patient care services. This role ensures clinical excellence, supervises interdisciplinary team members, and supports clinicians in providing outstanding patient-centered care.

Pay Range: $140,000 - $150,000 per year
Location: Concord office + in the field as needed
Job Type: Full-time

Key Responsibilities
