About the Job

General Summary of Position

Responsible for the daily intake of inquiry calls coordinating and evaluating potential transplant recipients coordinating patient care discharge from hospital and oversees follow-up care. Oversees and provides outpatient clinical care. Analyzes and reports program outcomes.

Primary Duties and Responsibilities

  • Handles intake calls from potential transplant candidates and/or referring physicians. Assesses patients for placement on the transplant waiting list. Coordinates evaluation with the patient/family and appropriate members of hospital the transplant team the patient's primary care physician and cardiologist and clinical consultants.
  • Evaluates collected data regarding the medical plan of treatment available social service programs and knowledge of caseload or group work.
  • Participates in clinical rounds meetings and conferences consultations and educational programs designed to increase knowledge and to improve patient care and staff performance: serve as a consultant to physicians and staff members regarding services in the specialty area. (e.g. development of critical pathway).
  • Develops and coordinates in-service educational programs for the Heart Transplant Program and supporting staff; develop therapeutic programs to assist patient and family adjust to problems associated with acute hospitalization.
  • Assists in the development of procedures and guidelines as needed.
  • Coordinates follow-up care of transplant recipients; ensures that surveillance tests are performed in timely manner.
  • Performs a variety of administrative duties related to program. (e.g. monitoring/analyzing department statistics and outcomes generating reports to meet internal and external reporting requirements establishing and maintaining case file of transplant recipients interfacing with other departments within hospital regarding other aspects of patient care and services coordinating non-hospital services for patients).
  • Supports various business requirements of hospital (e.g. managed care contracting outcomes reporting statistical analysis regulatory marketing finance and budgeting cycle).
  • Participates in and supports all regulatory required activities of the transplant program.

Minimal Qualifications
Education
  • Bachelor's degree in Nursing required

Experience
  • 1-2 years of progressively more responsible job-related experience in a clinical environment giving knowledge of heart transplant protocols and patient management responsibilities required

Licenses and Certifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia required

This position has a hiring range of

USD $89,065.00 - USD $162,801.00 /Yr.
