About the Role
We're growing our Family Medicine team and recruiting Family Physicians to join our North York clinic. This is a chance to build a long-term family practice with strong clinic support behind you, and to play a key role in growing longitudinal family medicine at this location as we work toward forming a new FHO+ model.
From day one, you will work closely with a multidisciplinary team to support patient-centered care. In the near term, the practice will focus on walk-in medicine as we build toward a FHO+ model, with a growing focus on longitudinal care as the model takes shape.
Full-time Family Physician - FHO+ Locations: North York (5292 Yonge St)
Responsibilities
Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence
Support your patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity
Apply your full scope to create thorough diagnoses and treatment plans which utilize our multidisciplinary team to support patients with all interconnected aspects of their health journey
Engage with our multidisciplinary team and internal stakeholders through on-site education sessions and care innovation initiatives across HealthOne
Qualifications and Skills
