- Join an established team of 1 independent APP
- RN triage team in place
- Primarily Medicaid and Medicare
- Laboratory services available
- Long-standing clinic serving as a healthcare anchor in the community
- Meaningful, high-value care for patients with significant social determinants of health
- Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.
- Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.\
