Overview

ANNUAL SALARY (DEPENDING ON SKILLS/EXPERIENCE): $145,000.00 - $160,000.40

We are seeking a dedicated and collaborative Advanced Practice Practitioner (Nurse Practitioner or Physician Assistant) to work closely with one of our Endocrinologist at our San Leandro and Dublin clinic locations. The ideal candidate will have a strong foundation in general medicine, drive to continually learn, be adaptable and energetic, and thrive in a team-based care model.

Location: San Leandro & Dublin

Responsibilities

Responsibilities:
  • The APP will provide follow-up and ongoing management care to patients with endocrine disorders, including diabetic patients
  • Medication management for glucose, blood pressure, and cholesterol control
  • Ongoing communication with referring clinicians
  • Developing expertise in diabetes and endocrine disorder management
  • Managing an assigned patient caseload in partnership with physicians and the healthcare team
  • Coordinating patient care over time and across healthcare settings to optimize health outcomes

Qualifications

Requirements:
  • Master of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required.
  • Baccalaureate in Nursing
  • Current CA licensure.
