SUBJECT OF EXAMINATION: TRAINING AND EXPERIENCE EVALUATIONREAD THE FOLLOWING INFORMATION THOROUGHLY.THIS INFORMATION WILL AFFECT YOUR FINAL GRADE! You will receive a grade and rank on this exam based upon your previous work experience - and how you document the required information. It is critical for you to follow the directions exactly as shown below.
Be specific; vagueness and ambiguity will not be resolved in your favor. Additional information will not be accepted after submission of application.
In order to receive full credit for your work experience as a Cytotechnologist you must indicate the dates (mm/yyyy) and numbers of your ORIGINAL LICENSE and ASCP registration on the application. If you instead note the date of your current registration then you will not be given credit for any work experience that pre-dates your current registration/limited permit.
If you do not do so you may not receive credit for all of your work experience; you can only receive credit for your experience covered under your current registration or limited permit.
You must also supply all of the information requested on the application for each position you list. If you do not, you may not receive credit for that work experience.
Pay particular attention to the following items:
- The dates of your employment must be in mm/yy format,
- Do not write "varied" or give a range of hours for "Hours Worked per Week."
Instead, estimate the number of hours worked in a typical week.
- Use your official civil service title where appropriate.
- Under "Duties," clearly differentiate between Cytotechnologist work experience and other work experience.
Also indicate the percentage of time that duties in the field of cytology
were performed.
Note regarding resumes: Most resumes do not contain the detailed information needed to evaluate your experience. Therefore, the submission of a resume - which is not required - is a supplement to a fully completed application, not a substitute for it. To receive credit for a job, basic information such as employer's address, name and title of supervisor, hours in the work week, final salary, reason for leaving, etc.
must be entered in the appropriate areas of the examination application.
IT IS THE RESPONSIBILITY OF THE CANDIDATE TO NOTIFY THIS COMMISSION, IN WRITING, OF ANY CHANGE OF NAME, MAILING/LEGAL/EMAIL ADDRESS, OR PHONE NUMBER: INDICATE EXAMINATION NUMBER AND TITLE, AND YOUR SOCIAL SECURITY NUMBER.
