Responsibilities
The Clinical Manager - Home Health leads clinical excellence and brings the right balance of operational oversight and hands-on leadership, ensuring that clinicians are supported, care transitions are seamless, and patients experience continuity across the entire care continuum. By being physically present in the field, this leader models clinical excellence, drives consistency, and connects the dots between referral, care delivery, and transition, which creates a more cohesive, patient-centered system.
- Manages clinical operations by driving care continuity and transitions with a focus of clinical excellence at the point of care
- Ensures quality patient care with attention to timeliness of starts of care and transition readiness
- Completes comprehensive OASIS review at all established time points
- Works to connect home health patients to hospice, when appropriate
- Effectively manages employees by hiring, onboarding, managing performance, and supporting growth and development
- Supports staff education and development with frequent in-field shadowing of direct care staff.
- Administers clinical care to patients, as needed, to maintain clinical skills, to assess particular patient requirements or as support to patient care staff.
- Monitors key performance indicators and creates an action plan to improve results.
- Partners with intake and scheduling teams to support timely, clinically appropriate admissions, including referral prioritization, start-of-care readiness, and resolution of clinical barriers to admission.
- Serves as the clinical escalation point for complex or high-risk referrals, supporting intake decision-making and ensuring appropriate clinician assignment.

Qualifications
REQUIRED QUALIFICATIONS
- Bachelor’s degree in professional clinical discipline (RN, PT, OT, SLP, MSW) from an accredited college or university, Master’s degree desired. Certification in specialty preferred.
- Two (2) years of experience in a direct supervisory role.
- Must have excellent interpersonal, communication and analytical skills
- Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.
PREFERRED QUALIFICATIONS
- Experience in health care setting with working knowledge of medical terminology, electronic medical record and billing systems/software
- Home care experienced desirable
Required Licensure/Certifications
- Must hold at least one active unencumbered professional license in either VT or NH at the time of hire. Employees must have active unencumbered professional licenses in both VT and NH within 30 days of their hire date.
- Valid driver’s license with a clean driving record
- Current car insurance, which meets minimum standards
- BLS within 90 days of hire
- OASIS certification required within one year of employment
