Job Summary

The Family & Children's Services (FCS) Clinical Manager is responsible for the clinical, operational, and administrative leadership for interdisciplinary pediatric and family support services, including skilled nursing, rehabilitation, and family support programs. The clinical manager will be the primary point of contact for state-administered, federally funded programs, namely Children's Integrated Services (CIS) and Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program. The Clinical Manager is responsible for program oversight, staff and clinical supervision, quality and performance improvement, regulatory compliance, resource management, and coordination of care across the continuum of family and pediatric services. The Clinical Manager serves as a clinical resource and mentor to staff while fostering collaboration among internal teams, community partners, and referral sources.

The Clinical Manager is accountable for program operations, staffing, clinical quality, regulatory compliance, care coordination, and performance outcomes. This role works collaboratively with internal departments, community partners, hospitals, primary care providers, social service agencies, and leadership to strengthen care transitions, address social determinants of health, and support complex patient populations through innovative care models. The Clinical Manager promotes a culture of accountability, continuous improvement, and staff engagement while advancing program goals and organizational priorities

Qualifications / Job Requirements

Education and Licensure
• Bachelor's Degree in Nursing, Occupational Therapy, Physical Therapy or other relevant health care discipline required.
• Master's Degree in Nursing, Rehabilitation Services, Public Health, Healthcare Administration, Social Work, or related field preferred.
• Current unrestricted Vermont Licensure in applicable clinical discipline.
• Valid driver's license and reliable transportation.

Preferred Experience, Skills, and Attributes
• Five or more years of progressively responsible clinical experience in pediatrics, family health, rehabilitation, home health.
• Three or more years of leadership, supervisory, or program management experience.
• Experience in care management, case management, population health, transitional care, community health, chronic disease management, or value-based care models.
• Knowledge of healthcare reimbursement models, care coordination practices, and community-based service delivery.
• Demonstrated experience leading interdisciplinary teams and managing complex patient populations.
• Strong understanding of quality improvement methodologies, utilization management, and performance metrics.
• Experience developing community partnerships and working collaboratively with hospitals, primary care practices, behavioral health providers, Childcare providers and community agencies.

Responsibilities

Program Leadership and Operations
• Provide leadership and oversight for Family & Children's Supports clinical programs, including nursing, rehabilitation, and family support services.
• Assess community and client needs and support development of programs and services that improve patient outcomes and access.
• Coordinate, monitor, and evaluate clinical services, program effectiveness, client satisfaction, quality outcomes, and staff engagement.
• Manage referrals and clinical resource allocation to ensure timely access to services.
• Assist in developing annual program goals, priorities, budgets, productivity expectations, and performance improvement initiatives.
• Participate as a member of agency leadership teams, committees, and strategic planning efforts.

Clinical Leadership
• Serve as the primary clinical resource for nursing, rehabilitation, and family support staff.
• Maintain knowledge of pediatric, family-centered, home health, rehabilitation, and community-based services.
• Support clinical decision-making, case consultation, and problem-solving for complex patient situations.
• Promote evidence-based practices and quality care standards across all disciplines.
• Ensure clinical documentation complies with organizational, state, federal, and payer requirements.
• May provide direct client services as needed to support operational needs and maintain clinical competency.

Staff Management and Team Development
• Directly supervise clinical staff, support staff, students, and contracted personnel as assigned.
• Recruit, interview, hire, onboard, mentor, coach, develop, and evaluate staff.

• Assess staffing needs and coordinate schedules, caseload assignments, coverage plans, payroll approvals, and time-off requests.

Administrative Responsibilities
