Qualifications
• The ideal candidate has 2-5 years of experience as a healthcare case manager (hospital, outpatient, home health of hospice), Nursing Home Billing, DFCS Medicaid Case Manager, or as a Medical Social Worker.
• Ability to conduct thorough consultations to assess Medicaid eligibility.
• Proven track record of processing Medicaid applications efficiently and accurately.
• Strong collaboration skills to work effectively with team members and healthcare professionals.
• Excellent communication skills for educating care facilities about our services.
• Ability to work with attorneys to develop comprehensive estate and long-term care plans tailored to client needs.
• Proficiency in coordinating client support for nursing home, assisted living, or in-home care placements.
