Job Summary

The Clinical Care Manager has a system wide view of the hospital and is responsible for oversight 24/7 central coordination of patient admission, discharges, hospital throughput and employee staffing.

The Manager collaborates and coordinates with multidisciplinary teams to facility a safe quality experience for patients through the continuum of care, and will serve as the primary liaison between nursing, service locations, support departments, other healthcare facilities and in partnership with physicians to ensure appropriate patient placement for effective, efficient patient-focused outcomes.

The Manager is responsible for budget/labor management, payroll, patient/customer satisfaction, employee engagement. Will work collaboratively with all members of the healthcare team to improve throughput, length of stay (LOS), and readmission reduction performance of the organization.

QUALIFICATIONS
  • Bachelor's Degree in a health related field
  • Master's Degree in a health related field Preferred
  • Six (6) or more year's job related experience including minimum three (3) years of Case management experience. One (1) year of supervisory experience
  • Registered Nurse in State of Georgia
  • Basic Life Support (BLS)
  • Case Management Certification Preferred
