The Ambulatory Care Pharmacist is responsible for providing direct patient care through medication management and disease state monitoring in outpatient settings. The role works collaboratively within a multidisciplinary care team, partnering with physicians, advanced practice providers, nurses, and patients to optimize therapeutic outcomes, ensure medication safety and effectiveness, and support the achievement of patient care goals.
JOB REQUIREMENTS
Experience:
Five years of clinical pharmacy practice experience in health system or ambulatory care settings.
Prior or current experience as an ambulatory care pharmacist, with a demonstrated track record of achieving positive patient outcomes through effective medication therapy management preferred.
Education:
Pharm D - Doctoral of Pharmacy from an accredited pharmacy program.
Strong knowledge of pharmacotherapy and evidence- based medicine.
Completion of a PGY1 and/or PGY2 residency in Ambulatory Care is preferred
Licensure & Certifications:
MAPHAR - Licensed MA Pharmacist or eligible for reciprocal licensure at time of hire.
BCPACP - Board of Pharmacy Specialtes in Pharmacotherapy or Board of Pharmacy Specialties in Ambulatory Care Pharmacy.
