Full Job Description
As a Pharmacist on the Health Care Quality Improvement team, you will be responsible for working with hospitals, nursing homes, pharmacies, physician offices, health systems and other healthcare providers to improve the safety and quality of medication use among Medicare beneficiaries. Potential quality improvement projects may include analyzing and interpreting drug utilization data, working directly with collaborating entities to prevent and reduce adverse drug events, improving the quality of medication reconciliation and high-risk drug management across care settings during transitions, and supporting other medication-related quality improvement initiatives. The Pharmacist role may support other departments within the organization on medication related tasks as needed. Duties to include but not limited to:
  • Maintain relationships with collaborating partners and other health care providers in the community.
  • Facilitate work with collaborating entities to develop and implement evidence-based quality improvement projects to improve medication safety.
  • Organize collaborator meetings including development of meeting agenda, meeting minutes and follow up to ensure completion of required activities to advance the project.
  • Maintain expertise in Medicare beneficiary benefits as related to drug utilization and assist in building and maintaining relationships with key stakeholders, as well as other states.
  • Provide pharmacy expertise as consultant to other quality improvement teams as needed.
  • Maintain documentation of all activities as per CMS requirements and IPRO policies and procedures, including preparation of CMS deliverables.
  • Develop and deliver educational presentations to groups of providers as necessary to facilitate quality improvement activities, including facilitation of small group quality improvement as the subject matter expert for 1:1 technical assistance.
  • Assist with data analysis and manuscript preparation suitable for publication.
  • Coordinate with the preceptor or precept Pharm D students for advanced pharmacy practice experience rotations. Including working directly with students to facilitate their learning of QIN-QIO work.
  • Administrative duties as required, including preparation of internal and external progress reports and deliverables and ad hoc report requests.
  • Accept new duties as assigned in response to changing department needs.

QUALIFICATIONS:
  • Strong knowledge base of clinical pharmacy and the principles and practice of quality improvement.
  • Excellent communication (verbal and written) skills, including the ability to interact effectively with physicians, nurses, other pharmacists, and government agencies.
  • Analytic skills or willingness to learn needed skills, including the ability to analyze and interpret drug utilization data.
  • Familiarity with electronic health records, e-prescribing, electronic order sets and clinical decision support, and related health information technology.
  • Organizational skills, including the ability to manage multiple tasks and relationships with multiple parties.
  • Ability to work well with supervision and as part of a team.
  • Ability to travel to various sites throughout the IPRO region per contract (est. 25% out of area travel).
  • Proficiency in Microsoft Office (including Word, Excel, PowerPoint, Outlook, and Teams) and familiarity with Salesforce.

EDUCATION & EXPERIENCE:
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree, with current licensure in any state, required.
  • Five (5) years of pharmacist work experience including one (1) year in quality improvement activities and/or pharmacy data analytic activities, required.
  • Experience with health informatics in multiple care settings and one (1) year in quality improvement activities and/or pharmacy data analytic activities, preferred.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. The annualized salary range for this position is $115,000.00 - $126,000.00. Actual salary range and /or hourly rate depends on a variety of factors including experience, education, credentials, location, and budget.
