Adult Nurse Practitioner-- Post-Discharge In Home Care (N Ft Worth/ Alliance, TX areas)
Key Responsibilities
• Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
• Review and interpret discharge summaries and progress notes.
• Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
• Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.
Qualifications
• Current Texas medical license and active Medicare provider number required.
• Nurse Practitioner certification in good standing.
• Ability to travel within a 35-mile radius from home to see patients throughout N Ft Worth, Alliance, Keller, Haslet, and surrounding areas.
• Strong communication, clinical judgment, and documentation skills.
• A compassionate, faith-driven approach to patient care.
Compensation:
• Base Salary: $115,000 - $135,000 (commensurate with experience)
• Quarterly Performance Bonuses available
