Job Summary This position is responsible for providing direct management and supervision for the program's Support Coordinators, engaging with families seeking services through the county including conducting intakes and VIDES screenings for waiver eligibility, and participating in MHSADS initiatives and work groups at the discretion of the Program and/or Senior Manager. Additionally, the Team Coordinator may also be asked to manage an individual caseload as required by the program.
Responsibilities of the position may include, but are not limited to:
- Training and onboarding of new staff.
- Conducting supervision and quality assurance measures with assigned Support Coordinators.
- Assessing eligibility and needs of individuals.
- Interviewing individuals and their families.
- Providing assistance and consultation to Support Coordinators during crisis intervention and stabilization.
- Developing and documenting person-centered plans using a strengths-based approach.
- Assuring quality service provision via chart reviews and live supervision.
- Maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention.
The Support Coordination Team Coordinator is responsible for providing high-quality customer service consistent with the mission, vision, and values of MHSADS. Success in this position requires effective oral and written communication skills, strong organizational and time management skills, and proficiency in using the established electronic health records and other software programs.
Hiring salary is commensurate with experience.
Minimum Qualifications Master's degree in a human services field and two (2) years of professional experience working with individuals who have a developmental disability diagnosis; or equivalent combination of education and experience.
Preferred Qualifications:
- Supervisory experience.
- Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid.
- Knowledge of community-based resources including integrated housing options and employment first principles.
- Program experience with individuals with developmental disabilities.
- Ability to communicate in Spanish using all modes of communication.
Job Contingencies and Special Requirements Employment is contingent upon successful completion of criminal background check, TB test, driving record check, and a valid driver's license in Virginia or regional state of residence.
