Job Requirements
Provides clinical pharmacy services and collaborates with healthcare providers to ensure medication therapy is safe, effective, and appropriate. Responsibilities include medication management, patient education, drug dosing, medication monitoring, and providing clinical guidance within an assigned specialty area. Serves as a pharmacy subject matter expert, supports quality improvement and clinical practice initiatives, and ensures compliance with pharmacy policies and regulatory requirements. Provides guidance to Pharmacy Technicians and may support staff training and education. Works independently within their area of expertise and may lead projects or provide pharmacy leadership support as needed.
Primary Responsibilities

Provides pharmaceutical care to patients including the promotion of optimal drug therapy and safe use of medications. Assesses patient-specific data to evaluate medication needs, verifies medication orders, and oversees the preparation, dispensing, distribution and monitoring of all medications.
Lead systemwide formulary management activities, including evaluation, maintenance, and optimization of medication use standards across all sites.
Conduct evidencebased clinical, safety, utilization, and pharmacoeconomic reviews to support formulary decisions, drug policy development, and therapeutic interchange strategies.
Develop, implement, and maintain medicationuse policies, prescribing criteria, and clinical guidelines in alignment with regulatory, accreditation, and system governance requirements.
Serve as a subject matter expert to the Pharmacy & Therapeutics (P&T) Committee, preparing clinical reviews, presenting recommendations, and supporting committee operations.
Collaborate with pharmacy leadership, physicians, nursing, and interdisciplinary teams to ensure safe, effective, and costefficient medication use across the health system.
Monitor medication utilization trends, drug spend, shortages, and supply disruptions; recommend mitigation strategies and stewardship interventions.
Partner with IT, informatics, and EHR teams to implement formulary changes, build medicationuse workflows, and ensure accurate medication information within Epic.
Support quality improvement initiatives related to medication safety, standardization, and clinical governance across inpatient, outpatient, and emergency care settings.
Provide education and communication to clinical teams regarding formulary updates, drug policy changes, and evidencebased therapeutic guidance.
Collaborates and consults with and provides education to pharmacy and medical staff, nurses, allied health professionals, patients and caregivers. Acts as a preceptor for learners as directed.
Manages care, quality, and costs by engaging in the systematic and continuous process of evaluating and improving the utilization of medications in a patient population in order to enhance the quality, safety, efficiency and cost-effectiveness of the medication use process.
Takes a leadership role in continuous quality improvement efforts and represents the department as an interdisciplinary interface in the assigned specialty areas.

Work Experience
Education & Experience - Required
Doctorate of Pharmacy Required (PharmD) unless graduated from U.S. School of Pharmacy in or before 2003 OR Foreign Equivalent.
