Home Health Care, Inc. is hiring a Full-Time RN Visit Nurse to serve patients throughout St. Paul and surrounding communities.

Focused time with your patient.

When you walk through the door, you're not responsible for an entire hallway.

You're responsible for the person in front of you.

That means noticing the wound that isn't healing quite right.

Catching a medication discrepancy.

Recognizing a change in condition.

Teaching a family member what symptoms to watch for.

Calling the physician before a manageable problem becomes an emergency.

And sometimes simply taking the extra few minutes to make sure your patient actually understands what's happening.

Those individual visits add up to better outcomes.

What Will Your Day Look Like?

You'll travel throughout the St. Paul territory, providing skilled nursing visits to patients recovering or managing their health at home.

Your visits may include:

  • Comprehensive nursing assessments
  • Wound assessment and wound care
  • Medication reconciliation and management
  • Injections and other skilled nursing procedures
  • Monitoring acute and chronic medical conditions
  • Patient and caregiver education
  • Identifying and communicating changes in condition
  • Coordinating with physicians and other providers
  • Collaborating with PT, OT, SLP, social work, and other members of the care team
  • Helping prevent avoidable emergency room visits and hospitalizations
  • Completing timely documentation in Homecare Homebase (HCHB)

You aren't simply checking a task off a list.

You're helping determine whether that patient can continue succeeding at home.

The Hospital Shows You the Patient. Home Health Shows You the Whole Picture.

A medication list might look perfect in the chart.

Then you get into the home and realize the patient has three different pill bottles for the same medication.

The discharge instructions might say everything was explained.

Then you discover the family doesn't understand the wound-care instructions.

The patient may say they're doing fine.

But your assessment tells you something has changed.

Home health puts your nursing judgment into a completely different context.

You're seeing the environment.

The support system.

The barriers.

The actual day-to-day reality of managing an illness or recovering from hospitalization.

And you're in a position to do something about it.

Who Does Well in This Role?

You don't need someone standing next to you to know what good nursing looks like.

You assess.

You ask questions.

You notice things.

And when something doesn't look right, you act on it.

We're looking for:

  • An active Minnesota Registered Nurse (RN) license
  • Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or another clinical environment
  • Strong assessment and critical-thinking skills
  • Confidence working independently in the field
  • Excellent patient and family communication
  • Strong organization and time-management skills
  • Ability to collaborate effectively with an interdisciplinary team
  • Reliable transportation and willingness to complete visits throughout the St. Paul territory

Previous home health experience is valuable, but we're also interested in experienced nurses from other settings who are ready to bring their clinical skills into the home.
