About the RoleShanadoa Home Health, a Medicare-certified agency serving Northeast Oklahoma for 19 years, is looking for an experienced RN to lead our VA Homemaker/Home Health Aide program. You'll oversee clinical quality, supervise our aide team, and serve as our main point of contact with VA partners. The goal is to grow the program so more Veterans receive timely, high-quality care at home.
Responsibilities- Supervise and support home health aides; address clinical concerns in the field
- Review aide documentation and schedules against authorized services
- Monitor authorizations and renewals to prevent lapses in care
- Conduct program audits and lead corrective action
- Complete monthly RN visits for VA clients until a dedicated visiting nurse is hired
- Build relationships with VA community care staff, social workers, and care coordinators
- Streamline the referral-to-admission process and grow Veteran census
Requirements- Current, unrestricted Oklahoma RN license (or compact)
- 3+ years of home health or community-based nursing experience
- Hands-on experience with VA community care referrals and authorizations
- Experience supervising aides or paraprofessional staff
- Valid driver's license, reliable transportation, and auto insurance
Preferred- Experience running or coordinating a VA Homemaker/Home Health Aide program
- Established relationships with Oklahoma VA facilities and care teams
- Military, Veteran, or military-family background
