Summary:
Provides clinical nursing care and care coordination for children and adolescents with complex rehabilitation needs in support of three pediatric physiatrists. Responsibilities include direct patient care, clinical triage, patient and family education, and coordination across specialties, therapies, schools, community resources, and equipment providers.
Incentive:
$10,000 Commitment Incentive paid out in 6 installments over 6 years.
Responsibilities:
1. Works collaboratively with an interdisciplinary health care team to assess and coordinate medical, functional, developmental, therapy, equipment, and psychosocial needs. Uses expertise and experience in specialty practice to facilitate patient care. Acts as a liaison between the patient, family and health care team to communicate patient care needs. Provides direct nursing care, including focused assessment, medication reconciliation, clinical monitoring, and support for clinic-based procedures. Monitors provider schedules to ensure optimal patient access.
2. Collaborates with health care team to provide information and resources to facilitate patient care. Provides education regarding diagnoses, medications, therapies, equipment, mobility, safety, and home care. Coordinates with primary care clinicians, specialists, therapists, schools, home health agencies, equipment providers, and community resources. Assists staff in the development of patient education materials for the patient population. Performs patient teaching related to specific topics and patient needs. Documents patient teaching in the medical record in the appropriate sections.
3. Collects data related to specialized patient population and analyzes trends. Enters data or coordinates its entry into databases and registries for benchmarking purposes. Identifies recurring barriers and participates in developing pediatric rehabilitation workflows and quality improvement initiatives. Participates in performance improvement activities to promote quality patient care. Monitors outcomes of care for patient population and collaborates with the interdisciplinary health care team on evaluation of outcomes.
4. Coordinates longitudinal care before and after clinic visits, following hospitalization, and during transitions between care settings. Coordinates and oversees pediatric rehabilitation multidisciplinary clinics. Documents care planning and communication with health care providers in the medical record. Collaborates with other services for sedated procedures such as botulinum toxin injections and alcohol blocks.
5. Promotes own professional development through attending in-services and presentations. Keeps current in practice through conferences, reading current literature and research. Role models professional nursing practice to others through use of the nursing process and professional behaviors
Preferences:
Pediatric, ambulatory, rehabilitation, neurology, orthopedics, developmental disabilities, or care-coordination experience preferred.
Knowledge of therapy services, durable medical equipment, community resources, insurance authorization, and electronic medical record workflows preferred.

Education Requirements:
• Graduation from a state-accredited school of professional nursing
Licensure/Certification Requirements:
• Licensed to practice as a Registered Nurse in the state of North Carolina.
Professional Experience Requirements:
• Five (5) years of professional nursing experience.
Knowledge/Skills/and Abilities Requirements:
Strong pediatric assessment, clinical triage, care coordination, communication, and organizational skills. Ability to work independently while collaborating with multiple physicians and an interdisciplinary team.
