Qualifications

Full Job Description
ResponsibilitiesThe RN Minimum Data Set (MDS) Coordinator is responsible for accurate and timely completion and transmission of MDS assessment of all residents in facility. Participates as an active member of the interdisciplinary team. Collaborates with the DON to provide accurate information for quality care of the resident, including audits and care conferences. QualificationsEducation High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required Completion of MDS Assessment training class within 6 months of entry into job required Bachelor's Degree in Nursing preferredExperience 2 years of long term care experience required Experience in completion of MDS assessments preferredLicenses and Certifications RN - Current and valid Registered Nurse license to work within the state of California required BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required Disclaimers  Pay ranges listed are an estimate and subject to change.
