Full Job Description
The Home Care Supervisor will work with a large non-profit organization providing healthcare services to the geriatric population of the Bay Area.

DUTIES / RESPONSIBILITIES:

1. Provides skilled observation, nursing assessment, development and implementation of appropriate home care treatment plans for participants with home care needs.
2. Provides support, supervision and training to geriatric aides and home care nursing staff in carrying out the home care treatment plan.
3. Teaches and counsels participants and family members regarding participants' care and safety.
4. Participates in interdisciplinary teamwork by offering clinical expertise in team planning and problem solving. Communicates effectively with participants, family and IDT Members.
5. In partnership with the Nurse Manager, advances the goals and objectives of the organization by developing supportive department specific performance goals, implementing quality improvement plans in department, participating in establishing and monitoring financial target and working collaboratively with other supervisors/managers on issues related to quality programming efficiency and cost effectiveness.
6. In partnership with the Nurse Manager, ensures delivery of quality services by overseeing the department's compliance with regulatory requirements, adherence to policies and procedures and lading continuous process
improvement and risk management efforts.
7. Collaborates with other supervisors/managers and department staff though effective communication, demonstrated leadership and facilitation of change opportunities.
8. Physical requirements include ability to lift/move up to 75 pounds occasionally and up to 50 pounds frequently.
9. Physical activities require the ability to stop, reach, stand, walk, push, pull, lift grasp, talk and hear, and repetitive motions of the wrist, hand and fingers.

Required Qualifications:

1. Graduate from an accredited college or university with either an Associate or Baccalaureate degree in nursing
and current California Registered Nurse license.
2. Two+ years' experience as an RN working with the geriatric population preferred
3. Two+ years' experience as an RN working in home care and or home health care preferred
4. Leadership or supervisory experience preferred
5. Ability to work as part of an interdisciplinary team with excellent communication and interpersonal skills.
6. Skilled facilitator and problem solver
7. Proficient in Microsoft Office
8. Experience with electronic health record documentation preferred
9. California Public Health Nurse Certification is desirable
10. Valid California driver license and be willing to drive and travel.
11. Current CPR certification
