Hours Per Week: 40 hours
Schedule: Monday - Friday 8am -4:30-pm, 1 weekend per quarter.
RESPONSIBILITIES - Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
- Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
- Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff
- REQUIRED QUALIFICATION: Nursing Diploma or ASN, Registered RN in State of NY, NY driver's License
PREFERRED QUALIFICATION: BSN
1 year of experience working with a frail or elderly population preferred.Availability of Motor Vehicle6 months- 1 year experience conducting UAS-NY Community Assessment preferred.EDUCATION:AS: Nursing (Required)
