Team 1 Covers The Northern Nassau Area; Bayville, Bethpage, Carle Place, Great Neck, Hicksville, Kings Point, Lake Success, Manhasset, Mutton town, Old Bethpage, Port Washington,
Roslyn and Westbury.
- 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details
Our Home Care RN’s:
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
- Provide care to the most upmost standard
Minimum Qualifications:
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse.
- Basic Life Support Certification
- Valid New York State Driver’s License and reliable transportation
Posted Salary RangeUSD $95,000.00 - USD $125,000.00 /Yr.
