full-time Registered Nurse Case Manager to serve patients in Greenwich and Stamford, Connecticut. We have an immediate opening for a compassionate RN who wants to make a meaningful difference through personalized care.
As a home health nurse, you'll assess patient needs, coordinate care, and help patients and families feel more confident managing their health. Each visit is an opportunity to support safety, independence, and better outcomes.
What You'll Do- Complete initial and ongoing assessments to identify patient care needs.
- Provide skilled nursing care according to individualized care plans and physician orders.
- Monitor patient conditions, communicate changes, and take appropriate action.
- Coordinate with physicians and the care team to support safe, continuous care.
- Educate patients and families about their care, health conditions, and safety.
- Help patients manage their health and reduce avoidable hospital visits.
- Protect patient and staff privacy in accordance with HIPAA.
What You'll Need- Current Connecticut license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- At least 1 year of nursing experience in a clinical care setting.
- Home health experience preferred, but not required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
