Full Job Description
What You Will Do:
Conduct initial and annual health assessments in a way that promotes individuals served health, safety, dignity, and respect.
Collaborate with individuals served to develop person-centered care plans.
Maintain a collaborative and professional relationship with the funding source, staff, and stakeholders to support the care of individuals served.
Educate individuals served and their families/supports on relevant health-related matters as appropriate.
Promote the use of InstED emergency department diversionary services with individuals served and their families/supports.
Maintain familiarity with One Care policies and procedures and adhere to any updates or changes.
Complete documentation in accordance with program policies and timeframe requirements, and complete any additional program-specific forms and documents as required in a timely manner.
Attend in-services, meetings, and training courses as assigned and scheduled, and keep mandatory trainings up to date.
Complete Critical Incident and Mandated Reporting processes as needed.
Who You Are:
High School Diploma or GED required.
Current Registered Nurse License by the Massachusetts Board of Registration; must maintain current RN licensure and certifications.
Demonstrated experience providing services to individuals with behavioral health and substance use disorders strongly preferred.
Bilingual Spanish and English speaking preferred.
