Registered Nurse (RN) / Case Manager to join our growing Home Care team. Working closely with our Director of Nursing, you will help coordinate high-quality care, support caregivers, and help clients remain safe and comfortable at home throughout Bedford, MA, and surrounding communities.

Your Role as a Registered Nurse (RN) / Case Manager:- Conduct in-home assessments and reassessments and develop or update individualized care plans.
- Provide ongoing clinical case management, follow-up visits, and timely documentation.
- Support and educate caregivers, respond to changes in condition, and help ensure care plans are followed.
- Serve as a trusted clinical liaison for clients, families, caregivers, physicians, and other healthcare professionals.
- Collaborate with the Director of Nursing, scheduling, and care management teams to support continuity of care.
- Help address client and family concerns and coordinate appropriate clinical follow-up.
- Partner with community referral sources and support smooth care transitions for new and existing clients.
What We're Looking For:- Active, unrestricted Registered Nurse (RN) license in Massachusetts.
- Two or more years of nursing experience; home care, geriatrics, private-duty care, or care-management experience preferred.
- Strong clinical judgment, communication, organization, and documentation skills.
- A compassionate, dependable professional who enjoys working with older adults, families, and caregivers.
- Valid driver's license, insured vehicle, and ability to travel locally to clients' homes.
