Full Job Description
Are you an experienced Registered Nurse looking for a role that offers meaningful patient relationships, professional independence, and flexibility outside of a traditional bedside setting?

Right at Home Boston Northwest is growing, and we're looking for a compassionate and organized Registered Nurse (RN) Case Manager to join our home care team.

This is a field-based home care position serving clients throughout our Boston Northwest service area, including Bedford, Lexington, Burlington, Woburn and surrounding communities. Local travel between client homes is required.

Working alongside our Director of Nursing and care team, you'll help oversee the care of older adults who want to remain safe and independent in their homes. This is an excellent opportunity for an RN who enjoys clinical assessment, care coordination, working directly with families and caregivers, and having more independence than a traditional bedside role.

What You'll Do
  • Conduct in-home nursing assessments and reassessments for new and existing clients
  • Develop, review, and update individualized plans of care
  • Provide ongoing clinical case management and follow-up visits
  • Monitor changes in client condition and coordinate appropriate clinical follow-up
  • Educate, coach, and support caregivers in delivering safe, high-quality care
  • Serve as a clinical resource for clients, families, caregivers, and our office team
  • Collaborate with the Director of Nursing, scheduling, and care management teams to maintain continuity of care
  • Communicate with physicians and other healthcare professionals when appropriate
  • Support clients and families during changes in care needs and transitions
  • Maintain accurate and timely clinical documentation
  • Build positive relationships with clients, families, caregivers, and community healthcare partners

Schedule & Flexibility

We are open to both full-time and part-time candidates.

Our preference is for an RN interested in 32-40 hours per week, but we don't want to miss a great nurse simply because they need a reduced schedule.

Qualified candidates seeking approximately 24 or more hours per week are encouraged to apply.

This is a mobile, field-based role involving local travel to clients' homes throughout Burlington and our broader Boston Northwest service area.

  • $45-$52 per hour, based on experience

What We're Looking For
  • Active, unrestricted Registered Nurse (RN) license in Massachusetts
  • 2+ years of nursing experience
  • Home care, geriatrics, private-duty nursing, or case-management experience preferred, but not required
  • Strong clinical assessment and judgment skills
  • Excellent communication and interpersonal skills
  • Strong organization, time-management, and documentation skills
  • Comfortable working independently while remaining part of a collaborative clinical team
  • Compassionate and dependable, with an interest in working with older adults and their families
  • Valid driver's license and insured vehicle
  • Ability and willingness to travel locally between clients' homes
