Role OverviewHome Health RN Case Manager Manito Home Health is seeking a
full-time Registered Nurse Case Manager to serve patients across Spokane and surrounding communities. This role is ideal for nurses who want autonomy, deeper patient relationships, and the ability to deliver high-quality care directly in the home setting.
Key ResponsibilitiesRN Case Manager Responsibilities- Complete comprehensive nursing assessments and develop individualized care plans
- Provide skilled nursing interventions in patient homes
- Manage a balanced caseload with strong organizational efficiency
- Coordinate care with interdisciplinary team members
- Educate patients and caregivers on disease processes, medications, and safety
- Maintain accurate, timely documentation
- Communicate effectively with physicians, families, and internal team members
- Support safe transitions from hospitals, SNFs, and outpatient settings
ScheduleFull-Time Schedule- Monday-Friday
- Daytime schedule
- Balanced caseload with strong team support
- Limited on-call rotation depending on team structure
QualificationsRN Requirements- Current Washington RN license (or ability to obtain)
- One year of nursing experience preferred; home-health experience a plus
- Strong assessment, communication, and critical-thinking skills
- Ability to work independently in a field-based role
- Reliable transportation and valid driver's license

Wage: $45-55 per hour DOE plus mileage reimbursement
