Full Job Description
Under the supervision of the Ambulatory Patient Care Manager, the Clinical Nurse II - Practice Nurse in the Hematology, Blood & Marrow Transplant and Cellular Therapy Clinic is responsible for providing direct and indirect clinical care to adult outpatients who have a variety of hematologic, immunologic, and oncologic conditions. Care is provided through the systematic application of the nursing process: assessment, planning, implementation and evaluation. All components of this process are individualized to meet the physiological, psychosocial and developmental needs of the patients. The Practice Nurse performs ongoing assessments and modifies the care planning process as needed, to ensure continuity of care between inpatient, outpatient, homecare, home, and multiple providers. The Practice Nurse is actively involved in patient, staff and provider education. (S)he participates in clinical research activities within the program and incorporates research findings into planning and implementation of care.
