Full Job Description
Our Community Care Internal Medicine Niskayuna office is looking for a full-time an RN to join their team! This is a full-time position. The hours are: Monday-Friday, 8:00AM-4:30PM.

**This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities:
  • Document verbal conversations with patients when appropriate.
  • Document information into patient record in an accurate, legible and timely manner.
  • Assess vital signs and the reason for each patient visit.
  • Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.
  • Support care management activities including coordination of patient needs, follow-up and connection to resources.
  • Assist with transitions of care - coordinating post-discharge follow-up, medication reconciliation and communication with patients and care teams.
  • Prepare patient for visit as appropriate, based on office/provider requirements.

Compensation: $32.00 - $45.50 per hour
