DescriptionRegistered Nurse / Case Manager V-Tech Solutions is seeking an experienced Registered Nurse / Case Manager to provide nursing, case management, and care coordination services to individuals with intellectual and developmental disabilities. Position Summary The RN / Case Manager will support individuals living in residential programs throughout Washington, DC, Montgomery County, and Prince George's County. Responsibilities include nursing assessments, care planning, medication management, healthcare coordination, staff training, documentation, and collaboration with families, physicians, and interdisciplinary teams.
Key Responsibilities - Conduct nursing assessments and develop individualized care plans.
- Coordinate medications, treatments, appointments, immunizations, and other healthcare needs.
- Supervise clinical functions of LPNs, CMTs, and TMEs.
- Maintain accurate health records and Health Passports.
- Communicate with individuals, families, physicians, and healthcare providers.
- Provide clinical guidance and health-related training to program staff.
- Participate in ISP, PCP, and interdisciplinary team meetings.
- Provide developmental assessments and coordinate community healthcare resources.
- Promote health, safety, independence, and continuity of care.
Required Qualifications - Minimum 3 years of Registered Nurse experience.
- Associate degree or equivalent in Nursing.
- Active RN license in Maryland and the District of Columbia.
- Delegating Nurse authorization in Maryland and DC, or willingness to complete required orientation.
- Experience as a Case Manager or Care Manager preferred.
- Experience working with individuals with intellectual and developmental disabilities is strongly preferred.
- Valid driver's license and reliable personal vehicle required.

Schedule & Location - Office location in Silver Spring, Maryland
- Service area includes Washington, DC, Prince George's County, and Montgomery County
- Travel between program locations is required.
Compensation - Mileage reimbursement provided.
Pay Range $86,000.00 Annually to $88,500.00 Annually
