Description
What You Will Do: The successful candidate will be willing and able to:
- Provide Medical Nutrition Therapy (MNT) and lifestyle medicine counseling for patients, specifically patients with chronic conditions such as Diabetes, Hypertension, and Obesity
- Perform comprehensive nutritional assessments and develop individualized care plans based on medical history and clinical data.
- Individually and/or with the nutrition team, plans, creates, and develops nutrition and health education topic outlines and curriculums, incorporating various learning techniques and formats (i.e., group discussion, lectures, role-play, etc.).
- Reviews and assists in determining the linguistic and cultural appropriateness of materials, teaching aids, activities, methods, strategies, programs, etc.
- Review the efficacy of materials and programs and provides positive, action-oriented feedback.
- Assists with the translation and/or revisions of translated materials as appropriate.
- Collaborate with clinical and non-clinical staff to integrate nutrition education into broader patient care strategies.
- Monitor and evaluate patient outcomes, providing consistent follow-up to ensure continuity of care.

Eligibility: Benefits are available to employees working
30+ hours per week, effective the first of the month following 30 days of employment.
Compensation Disclosure: $93,000.00 - $104,832.00The compensation range listed reflects Elica's good-faith estimate of the anticipated hiring range for this position in accordance with California pay transparency requirements. Final compensation will be determined based on job-related factors, including relevant experience, education, certifications, skills, and qualification.
Language Skills (Preferred):Bilingual or multilingual communication skills in English and
Russian, Farsi, Pashto/Pushto, or Spanish are preferred to support the needs of the communities we serve, where more than 50% of patients report a primary language other than English.
Requirements
- Registered Dietitian (RD) credentialed by the Commission on Dietetic Registration.
- Certified Diabetes Care and Education Specialist (CDCES) and/or Board Certified Specialist in Obesity and Weight Management (CSOWM).
- Minimum 2 years of professional experience, ideally within an FQHC or PCMH setting serving diverse populations.
- Active Basic Life Support (BLS) certification.
- Bilingual proficiency (Spanish, Russian, or Dari) preferred.
- Electronic Health Record (EHR) experience required, EPIC preferred.
- Strong clinical knowledge of medical terminology, ICD-10, CPT, and healthcare systems.
Additional Requirements
- Must have a current and valid California driver's license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours.

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required
by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet. The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine)
and hazardous chemicals.
