Position: Regional Clinical Support Specialist Social Worker

Compensation: $100,000 to $120,000 Offer dependent upon experience

Travel: 95% Travel required

JOB SUMMARY:

This role supports compliance, quality, and culture needs by honoring AccentCare's mission. The RCSS-SWer travels to designated sites as assigned social work clinical support and/or interim director of patient experience for the provision of social work services, clinical operations support of counseling professionals (social work, spiritual care, music therapy, child life), implementation and reinforcement of policies and procedures, and leadership coaching. This role provides direction to staff to ensure coordinated implementation of plans of care and the utilization of necessary services for comprehensive care of the patients that reflects the philosophy and objectives of both Palliative Care and Hospice. The role contributes to the AccentCare mission by coordinating and supporting the delivery of developmentally-appropriate psychosocial, spiritual, and psychoeducational care elements. This role is a member of the regional team(s) and acts as a subject matter expert for managing patient/client-facing care, including meeting support with patient experience disciplines designed to evaluate and achieve individualized site goals, and improve access to care and staffing pipelines as necessitated. Supervisory duties may include interns, social work fellows, and/or supervision for social workers towards clinical licensure. In conjunction with agency practices, , follows the standards of conduct and code of ethics set forth by the National Association of Social Workers (NASW)

FOUNDATIONAL RESPONSIBILITIES:

The RCSS-SW serves three key areas:

• Partnership as a leader in order to impact culture and how to balance the interdisciplinary team voice via coaching of site leaders
• Complete B2Bs (Back to Bedsides/Skills Checks/Competencies) to help the interdisciplinary team collaborate (i.e. RN and MSW joint visits)
• Train and support new social workers (and other counseling disciplines) to improve retention and support staffing pipeline initiatives.
  • Ensure all initial and subsequent assessments are conducted by qualified team members within required time, including documentation completion, submission and synchronization.
  • Ensure 24-hour availability of nursing and psychosocial services to patients and chosen families and coordinates on-call assignments, as necessary.
  • Oversee the support of development, implementation and review of interdisciplinary plan of care for each patient, executed at IDG sessions.
  • Collaborate with site leadership members to monitor, manage and mentor all staff in patient care activities, productivity and documentation, as related to applicable policies and protocols.
  • Participates in recruiting efforts via the selection and evaluation of all direct patient/client-facing care team members.

• Master's degree that meets state-specific requirements
• Must have current highest social worker licensure available within state of residency
• At least one year of experience as a clinical social worker post graduate
• Ability to travel 95% of the time
